HIPEC, (heated intraperitoneal chemotherapy) and debulking. This is a procedure we do for widely metastatic disease. It can sometimes be a last resort, to buy someone more time and give them a break from chemo. In other cases, like mucinous tumors, it can potentially be curable! With mucinous tumors, they are exactly how they sound - pockets of jelly spread throughout the abdomen. A few facts about HIPEC: - Patients must be optimally debulkable for maximum benefit. This means that we have to reduce the tumor amount down to only 2-3mm of thickness. We do this by removing bowel, stripping the peritoneum, peeling tumor off the liver, diaphragm, etc. - Hot chemo is then poured in the belly and agitated for 30 minutes. The chemo is warmed to 104-109F (optimum temp for destruction of malignant cells) - The patient’s body temp can rise to 102F, so anesthesia is always keeping an eye on them. - The drugs that we use have an established effect on malignancies, but low direct toxicity. . .
